That's James 5:16's instruction to confess to one another so healing can happen, stripped of its churchy phrasing and put in the first person.

"There is freedom found when we lay our secrets down at the cross" lines up with 1 John 1:7's claim that walking in the light brings fellowship instead of isolation — the opposite of the "plastic smile" the first verse describes. The title phrase repeated at the end of every chorus isn't a caveat; it's the whole condition the healing runs on, which is why the song keeps circling back to it instead of resolving into a tidy final verse.
